Rates and Insurance

Insurance:

I am in network with Preferred One and Blue Cross Blue Shield of MN. I am out of network for most other plans. Services may be covered in-full or in-part by your health insurance or employee benefit plan.

Please check your coverage with your insurance provider by asking the following questions:

Do I need a referral or pre-approval for mental health treatment?

What is my deductible and has it been met?
How much will I pay for services after I meet my deductible?
How many sessions per year does my health insurance cover?
What is the coverage amount per therapy session?

There is a sliding fee scale option for clients paying Fee-For-Service (Private Pay).

Payment:

All session fees are due at the beginning of each session. Cash, check or major credit card is accepted for payment. Please make checks payable to: Anne Delmont, LLC.

Cancellation Policy:

If you do not keep your scheduled appointment, and you do not give a 24-hour notification, you will be charged a late cancellation fee for the missed appointment.
